The Spiritual Status Questionnaire
On the following two pages is a questionnaire to help you determine where you currently stand in your relationship with the Lord and in your knowledge of the Bible. It is provided as a tool to help you evaluate areas in which you need to grow.
- Please read each question and circle the number on the scale—from “strongly agree” to “strongly disagree”—that best indicates where you stand at this moment.
- The questionnaire will be most helpful if you answer as honestly as possible. These answers are for your own personal use only. You will not be asked to share them with anyone. (However, once you develop a Spiritual Formation Plan based on your answers to these questions, we encourage you to share your goals with your Life Transformation Group or with one other person who can help you stay on track.)
- You do not need to tally your scores when you have finished. The next step is to go to the Spiritual Formation Plan at the end of this booklet. This worksheet will refer you back to the questionnaire and help you determine the classes that will be most beneficial to you at this time.
